Certified Registered Nurse Practitioner (HBPC)

The Home-Based Primary Care (HBPC) Program Nurse Practitioner is accountable for primary medical responsibility of the HBPC patients. This responsibility includes collaboration/consultation with other health professionals. The HBPC program deals with chronically ill Veterans with complex medical problems that cause them to be home bound; frequent interventions help them maintain their optimal level of functioning. The position is based out of the Johnstown, PA Community Based Outpatient Clinic (CBOC) and regularly travels to treat patient in their homes. Travel will be reimbursed in accordance with VA policy. Overnight travel is not expected. Functions or Scope of Assigned Duties:
The employee will perform home visits for Veterans in the HBPC program. The employee will also perform the initial, quarterly and annual Caregiver Program assessments. The employee initiates interdisciplinary projects to improve organizational performance. Serves on facility committees, and treatment team meetings as needed. Quality improvement activities include monitoring of patient satisfaction surveys, prescribed medical treatment adherence to all Joint Commission (JC) home care standards and clinical practice guidelines. Veteran's Equitable Resource Allocation (VERA) reimbursements, completion of all required documentation and timeliness of all treatment team meetings. Quality improvement model used is Plan, Do, Study, and Act. The employee uses professional standards of care and practice to evaluate programs and/or service activities. Shares knowledge and skill with others. Consultation - provides expert knowledge and guidance in Home Based Primary Care. The employee implements an educational plan to meet changing program or service needs for self and others. The employee maintains knowledge of current techniques, trends, and professional issues. Provides information and resources to services as needed. Attends local and national educational programs. The employee coaches colleagues in team building. Makes sustained contributions to health care by sharing expertise within and/or outside the medical center Facilitates discussion and problem solving on facility and ad hoc committees. Collaborates with other health professionals to ensure comprehensiveness and continuity of health care for patient care planning. The employee provides leadership in identifying and addressing ethical issues that impact clients and staff, including initiating and participating in ethical consultations. Maintains the confidentiality of patient information. Practices in a nonjudgmental, nondiscriminatory manner and is sensitive to diversity. Recognizes and resolves conflict of interest situations. The employee uses the group process to identify, analyze, and resolve care problems. Attends and actively participates in assigned committee and department meetings to convey information regarding monitoring activities. Attends and participates in meetings pertaining to focus evaluations, special projects, and task groups, as assigned. The employee collaborates with others to improve care. Participates in the writing or revision of policies and procedures as new information and research become available. The employee manages program resources (financial, human, material, or informational) to facilitate safe, effective, and efficient care. Incorporates the principles of fiscal responsibility when evaluating equipment and supplies related to HBPC program. Considers both clinical outcomes and financial implications when making recommendations for changes in HBPC. Work Schedule:
Tour of duty is varying daylight hours, Monday through Friday; however, weekend and evening hours may occur when Veteran needs arise. Travel to and from Altoona VAMC and other CBOCs as needed if staffing needs arise but at the time of travel the individual will be reimbursed according to local guidelines. This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. English Language Proficiency. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Nurse Practitioner Requirements:
Must be licensed or otherwise recognized as a Nurse Practitioner in a State Possess a Master's degree from a program accredited by the NLNAC or CCNE Maintain full and current certification as a Nurse Practitioner from the American Nurses Association or another nationally recognized certifying body. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
